The book focuses on the perception of senility in the Japanese mind and the attitude of elderly Japanese to the fact that as they age they begin to need help. Unlike in the Western world, where senility is seen as a disease, in Japan it is seen as a special condition called "bokeh". It resembles Alzheimer's disease and other forms of senile dementia, but bokeh is not clearly perceived as a pathology. According to the Japanese, it is a special type of disease that the patient can control to some extent. A study conducted by Trafagan in rural areas of the country shows how older Japanese people conceptualize bokeh and try to prevent or delay its onset.
