At the end of the last century, primary care physicians began to function as dispatchers whose main goal was to refer patients to a subspecialist. Over time, this situation was no longer satisfactory for doctors, patients and health care managers. In 1992 an order "On Phased Transition to the Organization of Primary Health Care on the Principle of General Practitioner (Family Physician)" was prepared and signed, which made it possible to introduce the principles of family physician work into primary health care.

The proposed manual was prepared by the teachers of the Department of Therapy, General Practice and Nuclear Medicine of N.I.Pirogov RNIMU with the active participation of doctors and teachers of the best school of family physicians - the University of Iowa (USA).
